Transitional Housing Program

The Father's Heart's Transitional Housing Program is a  hotel shelter initiative designed to support individuals and families in establishing a stable foundation for future housing.

 

We serve a diverse group of participants, ranging from families to single adults, all of whom are working towards long-term housing stability. We encourage our participants to be good neighbors and actively engage in their journey, regardless of their backgrounds. Please note that referrals to our program come exclusively through Clackamas County, as we cannot accept self-referrals.

 

​Our program is supported by four dedicated advocacy teams. The Case Management Team focuses on stabilizing clients while they are in shelter, addressing their immediate needs with comprehensive support. The Navigation Team assists participants in applying for and securing rental agreements, ensuring they have a solid plan for rent payments. Once housed, our Retention Team helps clients adjust to their new homes and maintain their leases, fostering a sense of security and community.

 

Additionally, the Care Coordination Team supports participants in navigating complex medical systems, ensuring they receive the best possible care throughout their journey.

Our structured approach not only addresses immediate housing needs but also promotes long-term stability and well-being for all participants. We are committed to creating an environment that reflects principles of love and grace, fostering a supportive community for the unique individuals we serve.
